Medication Forms

Before any medication will be given at school, two forms MUST be completed.

Medication Order -- This form contains information which MUST be provided by both the parent/legal guardian and the licensed provider.

Parent/Legal Guardian's Consent Form

Both forms must be completed and given to the school nurse before any medication will be given at school.
