Student Registration Forms

To enroll a student in Crescent Elementary and Jr. High, the following forms must be completed and turned in to the school's front office.

• Student Information Sheet for Registration
• State of Louisiana Health Information
• McKinney-Vento Confidential Referral Form
• Louisiana Student Residency Questionnaire Form
• Home Language Survey

You will also need to provide the following:

• Student's birth certificate
• Student's social security card
• Student's shot record
• Drop/Transfer sheet from the school in which the student is currently enrolled
• 2 proofs of residency

Information is available about sudden cardiac death in the young.  You may obtain this information by using the attached link below.
